COUNSELING ASSESSMENTS
Before arriving for your initial counseling appointment, you are asked to complete the Personality Style Assessment (PSA). Print each page and the answer sheet. Answer each question, keeping in mind that candid and honest responses will allow your counselor to accurately help you. Call for clarification if you have questions at (503) 281-3151.
Once completed, set the assessment materials in a confidential and safe place to bring to your first appointment.
The Personality Style Assessment is required of each person entering counseling. In the case of couples or families, each individual will complete the PSA. Click on the link above to view and print the PSA and answer sheet.
